Job Description

The Licensed Practical Nurse (LPN) will provide direct and indirect skilled nursing care for assigned residents, following defined policies and procedures under the supervision of a Registered Nurse. The LPN should have the capability to address the unique needs of adult and geriatric populations, ensuring the delivery of high-quality nursing care.

Responsibilities

• Collect ongoing data regarding residents' physical and psychosocial condition and effectively communicate findings to the RN and interdisciplinary team.
• Contribute to the planning of developmentally appropriate and culturally relevant resident care, ensuring involvement of the resident and family.
• Recommend care plan revisions based on resident outcomes and collaborate with the interdisciplinary team to meet all aspects of resident care.
• Maintain and enhance nursing practice knowledge through continuing education and in-service programs.
• Assist in the mentoring and orientation of new staff, promoting safety, cost-effectiveness, and positive patient outcomes.

Required Experience / Certifications / Licensure

• Current New York LPN Licensure.
• Basic Life Support (BLS) certification (AHA or RQI specifically).
• Minimum of 1 year experience as an LPN, preferably in a Long-Term Care (LTC) Facility.
