Part Time Nights (2x8s/3x8s), including every other weekend and holidays. Must be able to start on a Tuesday or Thursday.

Job Description

The LPN (Licensed Practical Nurse) must demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served on his or her assigned unit. This role requires the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements and provide care needed by adult and geriatric patient groups.

Responsibilities

• Collect and communicate ongoing data relevant to the resident's physical and psychosocial condition to the RN and other members of the interdisciplinary team.
• Contribute to the creation of a developmentally age-appropriate and culturally relevant plan of resident care reflecting nursing diagnosis and interventions.
• Recommending revisions to the plan of care after evaluating resident outcomes and indicating the need for referrals.
• Collaborate with the resident/family and team members to ensure customer satisfaction.
• Maintain current knowledge and competency in nursing practice and mentor/orient new staff.

Required Experience / Certifications / Licensure

• Current NY LPN licensure
• BLS (AHA or RQI only accepted)
• 1 year of experience serving in an LPN capacity at a long-term care facility
