Day shift - 5 days a week, 8-hour shift (08:00 - 17:00)

Job Description

We are looking for an experienced Home Health Registered Nurse (RN) to provide high-quality care to patients in their homes. The RN will perform skilled assessments, administer medications, and educate patients and their families on health management.

Responsibilities

• Provide skilled nursing care in patient homes including assessments and medication administration.
• Utilize medical equipment and adaptive devices to assist patients with their daily activities.
• Educate patients and their families about health management, medications, and home exercise programs.
• Document patient care and progress accurately as per regulatory requirements.
• Recognize and report signs of potential abuse or neglect.
• Ensure safety and implement injury prevention strategies for patients.

Required Experience / Certifications / Licensure

• Minimum of 1 year of home health experience required.
• Current state RN license or compact license required.
• BLS certification required.
• OASIS training preferred.
• Valid driver's license and a car for home visits.
