Physical Therapist - 13-Week Contract

Job Description

We are seeking a dedicated Physical Therapist to evaluate assigned patients and develop, implement, and modify treatment plans according to patient needs and physician direction. The successful candidate will be responsible for directing therapy services, ensuring the design of functional treatment programs for an effective return to community activities. This role also involves educating patients, families, students, and colleagues, while consistently upholding the organization’s Mission Statement and Core Values.

Responsibilities

• Conduct comprehensive evaluations of assigned patients and create tailored treatment plans focused on patient progress and discharge readiness.
• Collaborate with an interdisciplinary team to identify and address barriers to patient discharge.
• Manage individual, concurrent, and group treatment sessions effectively and efficiently based on patient care needs.
• Document all patient care activities clearly, accurately, and within established timelines.
• Participate in the education and oversight of students in the program, ensuring positive learning experiences.
• Engage in ongoing professional development and advocacy efforts within the community and profession.
• Adhere to state practice acts and maintain licensure in good standing, completing all required continuing education.

Required Experience / Certifications / Licensure

• BLS Certification is required prior to the first shift.
• A valid and current state license to practice as a Physical Therapist is required.
• A minimum of 2 years of experience in the field is required.

Schedule

This position is scheduled for day shifts, following a 5x8.0 schedule format.
