Job Description

The Licensed Practical Nurse (LPN) will provide direct and indirect skilled nursing care for assigned residents, demonstrating the knowledge and skills required for appropriate patient care. The LPN is expected to assess data reflective of patients’ conditions and provide care that meets the specific needs of adult and geriatric populations. This role will involve collaborating with nursing staff, physicians, and ancillary staff to ensure a comprehensive approach to resident care.

Responsibilities

• Collect ongoing data relevant to resident's physical and psychosocial condition and communicate findings to the Registered Nurse (RN) and other interdisciplinary team members.
• Contribute to developing culturally relevant resident care plans, actively involving residents and families in the process.
• Recommend revisions to care plans based on evaluations of resident outcomes and identify needs for referrals.
• Maintain current knowledge and competency in nursing practices and participate in mentoring and orientation of staff.
• Provide leadership in the professional practice setting under the direction of the RN, while considering safety, cost, and impact in care delivery.
• Adhere to safety protocols related to potential exposure to blood borne pathogens.

Required Experience / Certifications / Licensure

• Current NY LPN Licensure.
• BLS (AHA or RQI required).
• Minimum of 1 year of experience as an LPN, preferably in a Long-Term Care (LTC) facility.
