Licensed Practical Nurse (LPN) - Full Time Evenings

Schedule

Full Time Evenings, 5 days per week. Every Other Weekend and Holiday required.

Job Description

The LPN will provide direct and indirect nursing care for assigned residents, demonstrating the necessary knowledge and skills to care for adult and geriatric patient groups. The individual will follow established policies and procedures under the supervision of a Registered Nurse and collaborate with other healthcare professionals.

Responsibilities

• Collect ongoing data relevant to the resident's physical and psychosocial condition, communicating findings to the RN and interdisciplinary team.
• Develop culturally relevant care plans reflecting nursing diagnoses and interventions to achieve desired outcomes.
• Recommend revisions to the plan of care based on evaluations of resident outcomes and indicate needs for referrals.
• Maintain current knowledge and competency in nursing practices, participating in mentoring and orientation of staff.
• Possess the ability to provide care to geriatric residents while considering safety, cost, and impact in planning care delivery.

Required Experience / Certifications / Licensure

• Valid NY LPN Licensure
• BLS Certification (AHA or RQI required)
• Minimum of 1 year experience serving in an LPN capacity at a Long-Term Care Facility
