Day shift, 5x8-hour shifts (08:00 - 16:30). This facility cannot accommodate 4x10s or 3x12s.

Job Description

The Home Health Occupational Therapist will provide skilled therapy services to patients in their homes. The therapist will be responsible for evaluating and treating patients, developing treatment plans, and ensuring quality care within the community.

Responsibilities

• Conduct patient evaluations and develop individual treatment plans.
• Provide therapy interventions to enhance patient mobility and function.
• Document patient progress utilizing the EPIC/Oasis charting system.
• Collaborate with an interdisciplinary team to ensure comprehensive patient care.
• Visit up to 6 patients per day while maintaining productivity standards.

Required Experience / Certifications / Licensure

• Minimum of 1 year of skilled home health experience.
• Current BCLS certification is required.
• Experience with Oasis documentation is essential.
• Reliable transportation is necessary for patient visits.
• Must be fully vaccinated against COVID-19, including one booster.
