Day Shift: 5x8-Hour (08:00 - 16:30). Note: This facility cannot accommodate 4x10s or 3x12s.

Job Description

We are seeking a skilled Occupational Therapist to provide high-quality care in a home health setting. The ideal candidate will demonstrate expertise in developing treatment plans and conducting patient evaluations while offering compassionate support to patients in their homes. The therapist will have a manageable caseload with the potential to visit up to 6 patients per day.

Responsibilities

• Conduct patient assessments and develop individualized treatment plans.
• Provide therapy interventions according to established protocols.
• Document patient progress and update records in EPIC/Oasis.
• Encourage patient participation and provide education on home exercises.
• Coordinate with interdisciplinary teams to optimize patient care.

Required Experience / Certifications / Licensure

• Minimum of 1 year of skilled home health experience required.
• Active Occupational Therapist license and BCLS certification required.
• Experience with Oasis is essential; please specify previous experience on submission.
• Traveler expected to have reliable transportation.
