Schedule

Day shift, 5 days per week, 8-hour shifts (08:00 - 16:30). Please note that this facility cannot accommodate 4x10s or 3x12s.

Job Description

We are seeking an experienced Home Health Occupational Therapist to provide quality rehabilitative services to patients in their homes. This role includes assessing patient needs, developing individualized treatment plans, and implementing therapeutic interventions to assist patients in achieving their highest level of functional independence.

Responsibilities

• Conduct comprehensive assessments to determine patient needs and plan effective treatment.
• Implement treatment plans and therapy interventions focusing on rehabilitation goals.
• Document patient progress and adapt treatment plans as necessary.
• Educate patients and families on rehabilitation processes and techniques.
• Coordinate care with other healthcare professionals and services.

Required Experience / Certifications / Licensure

• Must have a valid Occupational Therapist license in Oregon.
• A minimum of 1 year of skilled home health experience required.
• Oasis experience is required; please provide details on previous usage.
• BCLS certification is mandatory.
• Reliable transportation is necessary.
• COVID-19 vaccination (complete primary series plus one booster) is required.
