Job Description

We are seeking a skilled Home Health Occupational Therapist to provide high-quality care to patients in their homes. The ideal candidate will be responsible for evaluating patients' needs, developing and implementing treatment plans, and assisting patients in achieving optimal functional outcomes.

Responsibilities

• Conduct comprehensive patient evaluations and develop individualized treatment plans.
• Provide therapy interventions to improve patients' daily living skills.
• Educate patients and their families on treatment plans and progress.
• Document patient progress and update care plans as needed using OASIS documentation.
• Manage a patient caseload of up to 6 patients per day, ensuring productivity standards are met.

Required Experience / Certifications / Licensure

• Valid Occupational Therapist license in the state of Oregon.
• Minimum of 1 year of skilled home health experience.
• Current BCLS certification.
• Experience with OASIS documentation is required.
