Day Shift: 5x8-Hour (08:00 - 16:30)

Job Description

We are seeking a dedicated Home Health Occupational Therapist to provide therapy services to clients in their homes. The ideal candidate will have experience in skilled home health therapy and the ability to deliver individualized care plans.

Responsibilities

• Conduct evaluations and develop individualized treatment plans.
• Provide therapeutic interventions in accordance with the plan of care.
• Educate clients and their families on treatment plans and therapeutic exercises.
• Document patient progress in compliance with agency policies and procedures.
• Manage a caseload of up to 6 patients per day while meeting productivity standards.

Required Experience / Certifications / Licensure

• 1 year of Skilled Home Health Experience is required.
• BCLS certification is required.
• OASIS experience is required.
• Reliable transportation is necessary for travel between patient homes.
• Primary series COVID vaccination and one booster are required.
