Job Description

The Home Health Registered Nurse will provide comprehensive home health care to patients in the North Princeton, NJ area. The ideal candidate must have recent and relevant home health experience, as well as proficiency in using EPIC and knowledge of OASIS documentation.

Responsibilities

• Conduct start of care and revisits for home health patients.
• Complete OASIS assessments and ensure accurate documentation in EPIC.
• Collaborate with interdisciplinary teams to develop and implement patient care plans.
• Monitor patient progress and adjust care plans as necessary.
• Educate patients and families on health management and home care procedures.

Required Experience / Certifications / Licensure

• Valid RN license in the state of New Jersey.
• Recent experience in home health care.
• Proficient in EPIC and OASIS documentation.
