Licensed Practical Nurse (LPN) - Mid Shift

Mid Shift: 5x8 hours from 15:00 to 23:00

Job Description

We are seeking a dedicated Licensed Practical Nurse (LPN) with experience in nursing facilities or nursing homes. The ideal candidate will possess strong skills in behavior management techniques and care for patients with degenerative disorders, including Alzheimer’s, ALS, and Parkinson’s. You will be responsible for providing compassionate and effective patient care while maintaining clear communication with the healthcare team.

Responsibilities

• Provide nursing care to patients in a nursing facility.
• Monitor vital signs and report changes to the supervising RN or MD.
• Assist with daily living activities, hygiene, and personal care.
• Implement behavior management techniques when necessary.
• Obtain 12 lead EKG/ECG as required.
• Participate in team meetings and multidisciplinary care planning.

Required Experience / Certifications / Licensure

• Active New York Single State LPN license (NY is not Compact).
• Current BLS certification.
• Minimum of 1 year of nursing experience, specifically in a nursing facility or nursing home.
• Experience with EPIC charting system is required.
