LPN - Long-Term Acute Care (LTAC)

Job Description

The LPN will be responsible for providing nursing care to patients in a Long-Term Acute Care facility. The successful candidate will work collaboratively within the healthcare team to ensure quality patient care while adhering to established policies and procedures.

Responsibilities

• Administer medications and treatments to patients as prescribed.
• Assist in patient assessments and evaluations.
• Perform patient care tasks, including vital signs monitoring and personal hygiene care.
• Assist with patient activities of daily living (ADLs) and provide support during rehabilitation efforts.
• Document patient care and progress accurately in the electronic medical record (EMR).
• Collaborate with healthcare team members to develop and implement patient care plans.
• Support the CNA duties as required, ensuring quality care is maintained.

Schedule

This role operates on an 8-hour evening shift. The LPN will be required to work a minimum of every other weekend and must be able to adjust their schedule to meet the needs of the unit and patient care.

Required Experience / Certifications / Licensure

• Current LPN license in New York State.
• A minimum of 1 year of LPN experience in a Long-Term Care setting is required.
• AHA BLS certification is required; additional certifications may be submitted for consideration.
• Background checks including a 7-year criminal history, FACIS 3, and NY State check must be provided at the time of submission.
