Job Description

The Case Manager is responsible for utilization reviews, resource management, discharge planning, treatment plan management, and financial management while completing medical record documentation. The role emphasizes patient and employee safety, ensuring high-quality care and adherence to safety standards.

Responsibilities

• Identify patient issues affecting their treatment adherence and assist families in navigating healthcare resources.
• Participate in interdisciplinary team meetings to create and manage clinical plans of care.
• Ensure thorough and timely documentation in medical records and maintain compliance with all regulatory standards.
• Manage utilization reviews and maintain consistent communication with payors to ensure satisfaction with case management services.
• Provide referrals for necessary social services and liaise with external agencies when appropriate.
• Implement discharge planning processes in collaboration with the clinical team to facilitate a safe transition to lower levels of care.
• Monitor and manage procedural and appointment needs according to the patient’s care plan.

Required Experience / Certifications / Licensure

• Current licensure in a clinical discipline (RN, RT, SW preferred) per state guidelines.
• Working knowledge of the insurance industry and government reimbursement mechanisms.
• Minimum of 2 years of experience in Case Management and Discharge Planning in an Acute Care setting.
