Physical Therapist - Rehabilitation

Job Description

The Physical Therapist will effectively screen, evaluate, and rehabilitate patients with physical functioning disorders and/or wound care, developing appropriate plans of care in compliance with all regulatory and clinical practice standards. Utilizing standardized assessments and evidence-based practices, the therapist will administer various procedures as part of the rehabilitation plan, including manual techniques, ambulation, therapeutic exercises, modalities, and wound care.

Responsibilities

• Assess patient progress and adjust treatment plans in compliance with regulatory and clinical practice requirements.
• Evaluate treatment outcomes for effectiveness and modify services accordingly.
• Provide consultation and counseling to patients, families, caregivers, and other service providers related to physical disorders.
• Communicate effectively with supervisors and other health team members regarding patient progress, barriers, and treatment plans.
• Document all regulatory and clinically required information in the electronic documentation system and maintain patient records according to company policy.
• Attend required meetings, including patient care conferences, utilization review meetings, and rehabilitation meetings.
• Supervise and support physical therapist assistants, rehabilitation aides, and/or students as needed.
• Adhere to APTA and state-required practice acts, codes of conduct, and ethics during professional practice.

Required Experience / Certifications / Licensure

• Minimum of 1 year of physical therapy experience; new graduates are welcome if they have Level II clinical experience.
• Long-term care (LTC) or rehabilitation experience is required.
• Current Oklahoma state license is required.

Schedule

This is a days shift, 8 hours per day position.
