Outpatient Physical Therapist

Job Description

We are seeking a dedicated Outpatient Physical Therapist to evaluate and provide treatment to a diverse patient population. This role involves developing, implementing, and modifying individualized treatment plans in alignment with patient needs, goals, and physician directives. The Physical Therapist will be responsible for guiding therapy services and ensuring effective return to community activities for patients.

Responsibilities

• Conduct comprehensive evaluations for assigned patients.
• Develop appropriate treatment plans and establish goals focused on patient progress.
• Document medical records in a clear and timely manner.
• Provide care under signed, valid physician orders, and ensure compliance with insurance requirements.
• Participate in student education programs, providing mentorship and supervision.
• Create and pursue a professional development plan with clinical management.
• Adhere to the discipline specific State Practice Act and maintain professional licensure.
• Engage in performance improvement initiatives to enhance service quality.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited school.
• 6 months to 1+ year of outpatient clinical experience.
• Current CPR certification.
• Active and applicable state licensure for Physical Therapy.

Schedule

This is a full-time position, operating on a 5x8.0 schedule, with assignments split between Hampden and Englewood.
