Job Description
The Outpatient Physical Therapist is responsible for evaluating assigned patients and developing, implementing, and modifying treatment plans in accordance with patients' needs, goals, and physician directives. The therapist will ensure the design of functional treatment programs to facilitate effective returns to community activities while serving as an educator to patients, families, and coworkers.
Responsibilities
- Conduct comprehensive evaluations of assigned patients and create tailored treatment plans focused on their discharge objectives.
- Document care in a clear, concise, and timely manner, ensuring compliance with state and federal guidelines.
- Participate in the student program by providing supervision and evaluations as needed.
- Actively pursue professional development and implement learned information to enhance service quality.
- Adhere to the State Practice Act and maintain licensure in good standing with all continuing education requirements.
- Contribute to performance improvement initiatives and offer feedback for maximized outcomes.
Required Experience / Certifications / Licensure
- Degree in Physical Therapy from an accredited institution.
- A minimum of 1 year of outpatient clinical experience.
- Current CPR certification.
- Active and applicable state licensure as a Physical Therapist.
- Previous assignments with our agency are highly desired, as is experience with Select Medical.
