Schedule
Tuesday - Saturday, 8 AM to 4 PM, with on-call rotation required.
Job Description
The Registered Nurse will provide hospice care to patients, ensuring adherence to evidence-based practices and compliance with State and Federal regulations. This role involves comprehensive patient assessment, care planning, and management of service delivery in accordance with the highest standards of quality care.
Responsibilities
- Complete OASIS documentation in line with agency policies, State and Federal regulations.
- Utilize the nursing process to assess patient and caregiver needs.
- Develop and communicate realistic care plans, including caregiver involvement.
- Manage caseloads effectively, adhering to agency productivity standards.
- Implement the medical plan of care, ensuring quality and safety standards.
- Stay current on high-tech skills and ensure infection control standards are maintained.
- Collaborate with LPNs, delegating tasks per scope of practice.
- Evaluate the effectiveness of care plans and make necessary adjustments.
- Maintain accurate and up-to-date patient documentation.
- Educate patients and caregivers on treatment options and care plans.
- Identify interdisciplinary needs for the patient and communicate effectively with other healthcare providers.
- Complete necessary agency and State forms accurately and promptly.
- Coordinate safe patient discharges and transfers.
- Participate in monthly peer review audits and case conferences.
- Advocate for patient care issues to ensure sensitivity to patient and family needs.
Required Experience / Certifications / Licensure
- Valid and current Registered Nurse license.
- Experience in hospice or home health care preferred.
- Knowledge of OASIS documentation requirements is essential.
