Registered Nurse (RN) - Home Health

Schedule

Full-Time, Monday to Friday with weekend rotation approximately every 4th week.

Job Description

This position is responsible for providing individualized goal-oriented nursing care using the nursing process in accordance with a physician’s order. In addition, this position serves as Case Manager in cases in which this position is the lead discipline.

Responsibilities

• Deliver high-quality nursing care in home health settings.
• Utilize the nursing process to assess, plan, implement, and evaluate care for patients.
• Manage and coordinate patient care as a Case Manager when designated as the lead discipline.
• Collaborate with healthcare team members to optimize patient outcomes.

Required Experience / Certifications / Licensure

• Current Registered Nurse (RN) license.
• Experience in home health or related field preferred.
