Job Description

We are seeking a dedicated Home Health Registered Nurse (RN) to join our team. This position is responsible for providing individualized, goal-oriented nursing care using the nursing process according to a physician's orders. As a Case Manager, you will be the lead discipline for designated cases, ensuring comprehensive care for our patients.

Responsibilities

• Deliver individualized nursing care to patients in their homes.
• Utilize the nursing process in executing care plans based on physician's orders.
• Serve as a Case Manager for cases where you are the lead discipline.
• Assess patient health and provide necessary interventions and education.
• Coordinate care with multidisciplinary teams to enhance patient outcomes.

Required Experience / Certifications / Licensure

• Active Registered Nurse (RN) license in the state of practice.
• Experience in home health or similar settings is preferred.
• Strong communication and interpersonal skills.
