Home Health Therapist Position

Monday to Friday, 8-hour shifts, totaling 40 hours per week.

Job Description

We are seeking a dedicated Home Health Therapist to provide exceptional therapy services in a home care setting within the regions of Freestone, Navarro, Limestone, Bosque, and Hill. The role involves conducting 4-6 visits per day, focusing on patient evaluations, treatment planning, and therapy interventions.

Responsibilities

• Conduct evaluations and develop treatment plans tailored to individual patient needs.
• Provide therapy interventions and monitor patient progress.
• Complete required documentation, including OASIS assessments.
• Maintain productivity standards of 28-32 points per week.
• Collaborate with healthcare teams to optimize patient care.

Required Experience / Certifications / Licensure

• Experience in Home Health and OASIS assessments is required.
• Familiarity with Homecare Homebase documentation is necessary; training may be provided for candidates without this experience.
• Valid licensure as a Physical Therapist, Occupational Therapist, or Speech-Language Pathologist.
