Outpatient Physical Therapist

Job Description

The Outpatient Physical Therapist evaluates assigned patients and develops, implements, and modifies treatment plans in accordance with the patients' needs, goals, and physician direction. The therapist is responsible for delegating and directing all discipline-specific therapy services for those assigned patients, ensuring functional treatment programs for effective re-integration into community activities.

Responsibilities

• Conduct comprehensive evaluations of assigned patients.
• Develop appropriate treatment plans and goals aimed at progressing patients towards discharge within an established length of stay.
• Document all medical records clearly, concisely, accurately, and timely, including goal writing and functional quality indicator assessments.
• Ensure care is provided under signed, valid physician orders and within insurance approval as necessary.
• Participate in the student program as assigned, providing positive learning opportunities for students.
• Actively pursue professional growth through continuing education and self-study.
• Maintain professional licensure in good standing and comply with all state-mandated continuing education requirements.
• Understand and apply performance improvement methodologies to enhance service quality.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited school.
• 1+ years of outpatient clinical experience.
• CPR certification.
• Active license for the state where work will be performed.

Schedule

This position operates on a 5x8.0 schedule.
