Part-Time Licensed Practical Nurse (LPN)

Schedule

PT Day/Evening variable shifts, scheduled as needed (2x8s/3x8s) + Every Other Weekend, Holiday (3x8.0)

Job Description

Under the direction of licensed nursing personnel, the Licensed Practical Nurse (LPN) performs direct and indirect resident care within the scope of practice defined by their certification. The LPN must demonstrate the knowledge and skills necessary to provide care tailored to the age and needs of the patients on their assigned unit, including the ability to assess data and interpret information to meet each patient's requirements.

Responsibilities

• Ensure the resident's Activities of Daily Living (ADLs) are addressed as outlined in the care plan, including hygiene, grooming, dressing, and toileting needs.
• Maintain the resident’s environment and safety in accordance with established policies and procedures.
• Meet the nutritional needs of residents as described in the care plan, including meal delivery and feeding assistance.
• Address resident's rehabilitative needs with proper mobility assistance, transfers, and use of assistive devices.
• Report residents' rights, refusals, and concerns promptly to supervisory staff, and recognize changes in their physical and emotional status.
• Adhere to safety protocols to minimize occupational exposure to bloodborne pathogens.

Required Experience / Certifications / Licensure

• Must hold a valid NY LPN License.
• BLS certification (AHA or RQI only) required.
• Minimum of 1 year of experience as an LPN in a long-term care setting.
• High School Diploma or GED is required.
