Outpatient Physical Therapist

Job Description

The Outpatient Physical Therapist is responsible for evaluating assigned patients and developing, implementing, and modifying treatment plans according to the patients' needs, goals, and physician direction. This role involves delegating and directing all discipline-specific therapy services for assigned patients, ensuring functional treatment programs for effective community reintegration. The therapist serves as an educator to patients, families, students, coworkers, and for personal development. They must comply with the organization's Mission Statement, Core Values, and Code of Conduct, emphasizing safety and quality in patient care.

Responsibilities

• Conduct comprehensive evaluations of assigned patients.
• Utilize clinical reasoning to develop appropriate treatment plans and goals.
• Maintain clear, concise, accurate, and timely medical record documentation.
• Provide care under valid physician orders and within insurance approvals.
• Participate in the student program by supervising and evaluating students.
• Create an individualized professional development plan with clinical management.
• Adhere to the discipline-specific State Practice Act and maintain professional licensure.
• Engage in organizational performance improvement initiatives.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited school.
• Minimum of 6 months of outpatient clinical experience.
• Current CPR certification.
• Active and applicable license for the state where work will be performed.
• Previous assignments with the agency or Select Medical are desirable.

Schedule

This position follows a 5x8.0 schedule, providing a structured work-life balance.
