Job Description

Seeking an experienced Home Health Physical Therapist to provide exceptional patient care in the Medford, OR area. The ideal candidate will work with a manageable caseload, providing personalized therapy to patients in their homes.

Responsibilities

• Deliver individualized therapy interventions based on evaluations and patient needs.
• Administer treatment plans to optimize patients' functional mobility and overall health.
• Conduct patient assessments and document care in the electronic health record (EPIC/Oasis).
• Provide education to patients and families regarding home exercise programs and rehabilitation techniques.
• Collaborate with interdisciplinary teams to ensure comprehensive patient care.

Required Experience / Certifications / Licensure

• Minimum of 1 year of skilled home health experience.
• Current Physical Therapist license for the state of Oregon.
• Basic Life Support Certification (BCLS) required.
• Experience with Oasis is required.
