Day shifts, 5x8 hours (08:00 - 16:30). Please note that this facility cannot accommodate 4x10s or 3x12s.

Job Description

We are seeking an experienced and dedicated Home Health Physical Therapist to provide exceptional care to patients in their homes. This position requires strong skills in patient evaluation, treatment planning, and therapy interventions. The ideal candidate will have a passion for helping others regain mobility and improve their quality of life.

Responsibilities

• Provide 1:1 therapy sessions to patients in their homes.
• May visit up to 6 patients per day while maintaining 30 productivity points standard.
• Conduct evaluations and create personalized treatment plans.
• Document services accurately and timely using EPIC Remote Client and Rover (Oasis).
• Collaborate with interdisciplinary teams to ensure comprehensive patient care.

Required Experience / Certifications / Licensure

• Must have 1 year of Skilled Home Health Experience.
• Oasis experience is required.
• Must hold valid OR and WA licensure.
• BCLS certification required.
• Must have own transportation; occasional maximum drive radius of 60 miles (60-90 minute total drive time) per day.
