LPN - Part Time (Day and Evening Shifts)

Part-time shifts are available during the day and evening, scheduled as needed by the facility, generally consisting of 2x8s or 3x8s, with weekend and holiday shifts required.

Job Description

The Licensed Practical Nurse (LPN) will demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served on their assigned unit. The individual will assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to their age-specific needs, particularly focusing on adult and geriatric patient groups.

Responsibilities

• Collect data relevant to resident's physical and psychosocial condition on an ongoing basis and communicate findings to the RN and other interdisciplinary team members.
• Contribute to the creation of a developmentally age-appropriate and culturally relevant plan of care reflecting nursing diagnosis and interventions to achieve desired resident outcomes.
• Recommend revisions to the plan of care after evaluating resident outcomes and indicate the need for referrals to ensure all aspects of the resident's care have been addressed.
• Maintain current knowledge and competency in nursing practice and participate in the mentoring/orientation of staff.
• Provide leadership in the professional practice setting under the direction of the RN and demonstrate knowledge and competency.

Required Experience / Certifications / Licensure

• Valid NY LPN Licensure
• BLS certification (AHA or RQI)
• Experience serving in an LPN capacity at a long-term care facility (minimum 1 year preferred)
