This is a 13-week contract position.

Schedule

Various schedules available, including:

• 5x8: 8-9 hour shifts Monday and Thursday, with 4-6 hours on Friday
• 4 x 10 hour shifts
• 3 x 10 hour shifts with 2 x 5 hour shifts

Job Description

The Outpatient Physical Therapist will evaluate all assigned patients and develop, implement, and modify treatment plans in accordance with patient needs, goals, and physician direction. Responsibilities include delegation and direction of discipline-specific therapy services for assigned patients and ensuring the design of functional treatment programs that facilitate a return to community activities. The therapist will also serve as an educator for patients, families, students, and coworkers and perform other professional duties associated with patient care and development of therapy services.

Responsibilities

• Complete comprehensive evaluations of assigned patients.
• Develop appropriate treatment plans and goals to progress patients towards discharge.
• Ensure accurate and timely medical record documentation.
• Participate in student program assignments, providing supervision and evaluation.
• Engage in professional development activities and apply learned information to meet strategic objectives.
• Adhere to State Practice Act and maintain professional licensure.
• Contribute to organizational performance improvement initiatives.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited school.
• 1+ year of outpatient clinical experience.
• Current CPR certification.
• Active and applicable state licensure for Physical Therapy.
• Previous successful assignments with the agency are highly desired.
