Schedule
Monday 8 AM - 5 PM, Tuesday/Thursday 9 AM - 7 PM, Wednesday 7 AM - 4 PM, Friday 7 AM - 1 PM
Job Description
The Outpatient Physical Therapist evaluates all assigned patients and develops, implements, and modifies treatment plans in accordance with the patient’s needs, goals, and physician direction. This role is responsible for delegating and directing all discipline-specific therapy services for those assigned patients, ensuring the design of functional treatment programs to allow for effective return to community activities. The therapist serves as an educator to patients, families, students, coworkers, and themselves, and performs other professional duties associated with patient care and program delivery.
Responsibilities
- Completes a comprehensive evaluation of assigned patients.
- Develops appropriate treatment plans and goals aimed at progressing patients towards discharge within an optimal timeframe.
- Documents all aspects of care in a clear, concise, accurate, and timely manner.
- Provides care under valid physician orders and ensures compliance with insurance requirements.
- Participates in student programs, providing a positive learning environment and supervision.
- Maintains up-to-date professional licensure and completes continuing education requirements.
- Implements performance improvement methodologies to enhance service quality.
Required Experience / Certifications / Licensure
- Degree in Physical Therapy from an accredited school.
- 1+ year of outpatient clinical experience is required.
- Current CPR certification.
- Active and applicable license for the state where work will be performed.
- Experience with Select Medical or previous assignments with the agency is highly desired.
