Day Shift: 5x8 Hours (08:00 - 16:30)

Job Description

The Home Health Physical Therapist will provide direct patient care to clients in their homes, focusing on rehabilitation and recovery. The role involves assessing patient needs, developing individualized treatment plans, and ensuring optimal patient outcomes through evidence-based practices.

Responsibilities

• Conduct evaluations and develop treatment plans for patients.
• Implement therapy interventions to improve patients' physical abilities.
• Monitor patients' progress and adjust treatment plans as necessary.
• Educate patients and their families on home exercise programs.
• Collaborate with other healthcare professionals to provide comprehensive patient care.

Required Experience / Certifications / Licensure

• One year of skilled home health experience required.
• Current BCLS certification.
• OASIS experience is required.
• Valid PT license in the state of Oregon.
