Job Description

A Home Health Physical Therapist is needed to provide individualized care to patients in their homes within the Medford, Oregon area. The position involves delivering high-quality physical therapy services that align with established standards to promote patient improvement and wellness.

Responsibilities

• Conduct evaluations and develop treatment plans for patients.
• Deliver therapy interventions and monitor patient progress.
• Utilize OASIS for documentation and care planning.
• Educate patients and families on exercises and care techniques.
• Manage a caseload of up to 6 patients per day, adhering to productivity standards.

Required Experience / Certifications / Licensure

• 1 year of skilled home health experience required.
• Active physical therapist license in the state of Oregon.
• Basic Cardiac Life Support (BCLS) certification required.
• OASIS experience is essential.
