Licensed Practical Nurse (LPN) - Long Term Care

Schedule

Evening shift from 2:30 PM to 10:30 PM, with every other weekend required.

Job Description

We are seeking an experienced Licensed Practical Nurse (LPN) for a long-term care position. The candidate will provide quality nursing care to patients in a 40-bed unit, catering to diverse populations including those with multiple sclerosis, orthopedic conditions, dementia, and memory care needs.

Responsibilities

• Deliver direct patient care, including administering medications and performing treatments.
• Assist in the daily living activities of residents.
• Monitor and document patient conditions in an EMR system (PointClick Care).
• Collaborate with healthcare team members to optimize patient outcomes.
• Float to all units within the same building as needed.

Required Experience / Certifications / Licensure

• Minimum of 1 year of LPN experience.
• Current New York State LPN license.
• Basic Life Support (BLS) certification.
