Licensed Practical Nurse (LPN) - Full Time Nights

Schedule

This is a full-time position with night shifts from 11 PM to 7:30 AM, working five 8-hour shifts weekly. This role includes every weekend and holiday shifts as scheduled.

Job Description

The Licensed Practical Nurse (LPN) is responsible for delivering direct and indirect nursing care to residents, ensuring the quality of care appropriate to the age and condition of the patients served. The ideal candidate will possess the ability to assess, interpret, and address the needs of adults and geriatric patients effectively.

Responsibilities

• Collect data related to residents' physical and psychosocial conditions and communicate findings to the Registered Nurse and other interdisciplinary team members.
• Contribute to the development of age-appropriate and culturally relevant nursing care plans, involving the resident and their families in the care process.
• Recommend adjustments to care plans based on evaluations of resident outcomes and ensure all aspects of residents' care are addressed.
• Maintain up-to-date knowledge and competency through participation in continuing education and in-service programs.
• Assist in mentoring and orienting new staff members, fostering a collaborative and safe care environment.

Required Experience / Certifications / Licensure

• Valid NY LPN Licensure.
• Basic Life Support (BLS) certification.
• A minimum of 1 year of experience serving in an LPN capacity, preferably in a Long-Term Care (LTC) facility.
