Job Description

The Outpatient Physical Therapist evaluates all assigned patients and develops, implements, and modifies treatment plans tailored to each patient's needs in accordance with their goals and physician direction. The therapist is responsible for the delegation and direction of all discipline-specific therapy services for assigned patients, ensuring the design of functional treatment programs that facilitate an effective return to community activities.

Responsibilities

• Conduct comprehensive evaluations of assigned patients and utilize clinical reasoning to develop treatment plans and goals focused on progressing towards discharge.
• Complete medical record documentation accurately and timely, ensuring compliance with state, federal, and organizational guidelines.
• Provide leadership in the delegation of therapy services and the implementation of individualized treatment plans.
• Participate in student programs, providing a positive learning environment through supervision and evaluations.
• Engage in self-directed professional development and apply acquired knowledge to enhance organizational objectives.
• Maintain professional licensure in good standing, adhering to ethical and regulatory standards.
• Contribute to performance improvement initiatives and follow through with action plans to maximize patient care outcomes.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited institution.
• 6 months to 1+ year of outpatient clinical experience required.
• Current CPR certification.
• Active and applicable state licensure.
