Registered Nurse (RN) - Care Coordination

Assignment Duration

This is an 8-week contract position, with an immediate start date.

Schedule

Day shift, 4x10 hours (07:30 - 18:00), including every other weekend.

Job Description

We are seeking an experienced Registered Nurse (RN) with a strong background in care coordination and discharge planning within an acute care setting. The successful candidate will work across various areas of the hospital, requiring versatility and adaptability.

Responsibilities

• Coordinate care and discharge plans for patients, ensuring a smooth transition to the next level of care.
• Collaborate with interdisciplinary team members to provide optimal patient outcomes.
• Manage multiple patient cases with a focus on effective care transition strategies.
• Utilize the EPIC charting system for documentation and patient care management.

Required Experience / Certifications / Licensure

• Minimum of 1 year of acute care experience is required.
• Experience in discharge planning and care coordination is essential.
• Current RN licensure and BLS certification are required.
• Experience working in a Level II Trauma Center is preferred.
• Must be willing to float between units, including Colby and Pacific campuses.
• References from at least two supervisors covering the past year are required.
