Registered Nurse (RN) Case Manager - Acute Care

This is a 13-week contract position.

Schedule

Day shift, 4x8 hours (07:30 - 16:00). Must work every other weekend.

Job Description

We are seeking a dedicated Registered Nurse (RN) skilled in case management and care coordination within an acute care setting. The ideal candidate will provide support in discharge planning and have a strong background in diverse case management roles.

Responsibilities

• Conduct thorough assessments and implement comprehensive care plans.
• Coordinate discharge planning, ensuring smooth transitions for patients.
• Collaborate with interdisciplinary teams to optimize patient care.
• Float between various departments within the hospital as necessary.
• Maintain accurate documentation using the EPIC charting system.

Required Experience / Certifications / Licensure

• Minimum of 1 year of acute care experience is required.
• BLS certification is mandatory.
• Current RN licensure is required (WA RN or Compact RN accepted).
• Experience in discharge planning and care coordination is essential.
• Prior experience in diverse case management roles preferred.
• Ability to work effectively in a fast-paced, dynamic environment.
