Registered Nurse (RN) - Care Coordination

Schedule

Day Shift: 5x8 Hours (07:30 - 16:00), with every other weekend required.

Job Description

The Registered Nurse (RN) will work in various areas of the hospital, focusing on care coordination and discharge planning within an acute care setting. The role requires collaboration with interdisciplinary teams to ensure effective patient transitions and manage lengths of stay.

Responsibilities

• Provide care coordination and discharge planning for patients across multiple units, including Med Surg and Emergency Department.
• Collaborate with healthcare professionals to develop and implement individualized care plans.
• Utilize Epic charting system to document patient information and updates.
• Assist in the identification of patient needs and mobilize resources for effective care.
• Facilitate smooth transitions of care for patients to ensure optimal outcomes.

Required Experience / Certifications / Licensure

• Current RN Licensure.
• Basic Life Support (BLS) certification.
• Minimum of 1 year of experience in acute care settings.
• Experience with discharge planning and care coordination is preferred.
• Willingness to float between units as needed.
