Case Manager - Day Shift

This is a 13-week contract position.

Schedule

Day shift, 5x8-hour schedule (08:00 - 16:30) with 2 weekends required every 6 weeks.

Job Description

The Case Manager will be responsible for overseeing patient care management, coordinating services, and ensuring that patients meet their treatment goals. This role requires effective communication with interdisciplinary teams to foster the best possible outcomes for patients.

Responsibilities

• Coordinate patient care and discharge planning with healthcare teams.
• Conduct assessments and develop treatment plans in collaboration with the medical team.
• Facilitate family meetings and manage resources effectively.
• Ensure compliance with Medicare, Medicaid, and other regulatory guidelines.
• Participate in case management meetings and contribute to process improvements.

Required Experience / Certifications / Licensure

• Minimum of 2 years of experience in case management.
• Experience with Epic charting system is required.
• Preferred experience in Long-Term Acute Care (LTAC) and Trauma Level I/II facilities.
• Certification in Case Management (CCM) is a plus.
