Job Description

The Registered Nurse (RN) will focus on Discharge Planning and Care Coordination in an acute care environment. The role involves collaborating with interdisciplinary teams to ensure effective patient transitions and optimal outcomes.

Responsibilities

• Provide comprehensive discharge planning and care coordination for patients.
• Work across various units within an acute care facility.
• Coordinate with social services and healthcare teams to facilitate patient needs.
• Support patients in navigating healthcare services post-discharge.

Required Experience / Certifications / Licensure

• 1 year of Acute Care experience is required.
• Experience in Discharge Planning/Care Coordination is necessary.
• Preferred background in diverse case management experience (Med/Surg, LLOS, ED).
• Current BLS certification and an active RN Licensure are mandatory.
