Day shift, 5x8 hours (08:00 - 16:30). This facility cannot accommodate 4x10s or 3x12s.

Job Description

We are seeking a qualified Home Health Physical Therapist to provide high-quality care to patients in their homes. The position requires a dedicated professional with strong skills in patient assessment and therapy, capable of working independently and managing their schedule effectively.

Responsibilities

• Provide one-on-one patient care in a home setting.
• Develop and implement effective treatment plans for a diverse patient population.
• Collaborate with healthcare professionals to optimize patient outcomes.
• Document patient progress using EPIC/Oasis charting systems.
• Ensure compliance with safety and health regulations.

Required Experience / Certifications / Licensure

• Minimum of 1 year of skilled home health experience.
• Current BCLS certification required.
• Experience with Oasis charting is essential.
• Strong communication and organizational skills.
• Must have reliable transportation and be able to cover a maximum radius of 50+ miles occasionally.
