Schedule

Day shift, 5 days a week, 8-hour shifts (08:00 - 16:30). This facility cannot accommodate 4x10s or 3x12s.

Job Description

We are seeking a dedicated Home Health Occupational Therapist to provide care to patients in their homes. The ideal candidate will have experience in skilled home health settings and be able to work independently while managing a diverse patient load.

Responsibilities

• Provide one-on-one therapy to patients in their home environment.
• Assess and document patient progress in accordance with best practices.
• Design and implement treatment plans tailored to individual patient needs.
• Communicate effectively with patients, families, and interdisciplinary team members.
• Complete OASIS documentation as required.

Required Experience / Certifications / Licensure

• Minimum of 1 year of skilled home health experience required.
• Oregon Occupational Therapy license is required.
• Current BCLS certification is required.
• OASIS experience is preferred.
• Reliable transportation needed for client visits.
