LPN - Part Time Evenings

Schedule

This position requires part-time evening shifts consisting of 2 shifts of 8 hours one week and 3 shifts of 8 hours the following week, including every other weekend and holiday as needed.

Job Description

The LPN will provide direct and indirect skilled nursing care to assigned residents in accordance with the defined policies and procedures under the supervision of a Registered Nurse. The individual will be responsible for providing care that meets the unique needs of adult and geriatric patients.

Responsibilities

• Collect ongoing data relevant to residents' physical and psychosocial conditions, and communicate findings to the RN and interdisciplinary team.
• Contribute to the development of culturally relevant and age-appropriate care plans involving the resident/family and healthcare team.
• Evaluate resident outcomes and recommend revisions to the plan of care to ensure comprehensive attention to residents' needs.
• Maintain current knowledge and competency in nursing practice and participate in the mentoring and orientation of new staff.
• Demonstrate leadership under the direction of the RN while ensuring safe and effective care delivery.

Required Experience / Certifications / Licensure

• Current NY LPN Licensure
• BLS certification through AHA or RQI
• A minimum of 1 year of experience as an LPN in a long-term care facility
