Job Description

The Outpatient Physical Therapist will evaluate assigned patients and develop, implement, and modify treatment plans in accordance with each patient's needs, goals, and physician directives. As part of the role, the therapist will also design functional treatment programs to facilitate the effective return of patients to their community activities. The therapist will serve as an educator to patients, families, students, coworkers, and themselves, ensuring the delivery of superior patient care aligned with the organization’s mission and core values.

Responsibilities

• Complete comprehensive evaluations and develop appropriate treatment plans with focused goals for patient progress.
• Maintain clear, accurate, and timely medical record documentation in compliance with applicable regulations.
• Participate in student programs, providing a constructive learning environment and completing evaluations.
• Create an individualized professional development plan in collaboration with clinical management.
• Ensure adherence to state practice acts and maintain professional licensure in good standing.
• Demonstrate awareness of performance improvement initiatives and contribute feedback for service enhancements.

Required Experience

• Degree in Physical Therapy from an accredited institution.
• Minimum of 1 year of outpatient clinical experience.
• Current CPR certification.

Certifications/Licensure

Active Physical Therapist license required.
