Schedule/Shift

8:00 AM - 4:00 PM, every other weekend, one summer and one winter holiday. Comp days provided during the week when working the weekend.

Job Description

The Registered Nurse (RN) in this Home Hospice role will be responsible for performing comprehensive patient assessments to evaluate their appropriateness for hospice care. This includes understanding patient and family learning needs and progress toward established goals. The RN will develop and implement individualized care plans based on evidence-based knowledge and collaborate with interdisciplinary teams to communicate any adjustments in patient care plans.

Responsibilities

• Conduct patient assessments to determine hospice care suitability and track progress towards care goals.
• Utilize services efficiently while adhering to visit productivity standards and insurance reimbursement knowledge.
• Maintain documentation through timely laptop data transfers and ensure compliance with discharge protocols.
• Employ age-appropriate teaching methods for patients and families, revising education plans as necessary and documenting tools used.
• Manage case responsibilities, revising care plans based on patient needs and hospice regulations.
• Ensure a safe healthcare environment by following infection control measures and managing medication needs effectively.
• Participate in monthly staff meetings and performance improvement initiatives, including data gathering for reviews.
• Assist with onboarding new employees and participate in cross-training activities.

Required Experience

• Two (2) years of experience as a Registered Nurse in a home hospice environment.
• Experience with electronic medical records (EMR) and strong critical thinking and communication skills.

Certifications/Licensure

• Current New Jersey RN License.
• Valid New Jersey driver's license.
• BLS certification.
