Job Description

The Registered Nurse will provide patient assessments, develop care plans, and ensure effective use of hospice services. Responsibilities include coordinating interdisciplinary communication, prioritizing patient care needs, and participating in performance improvement initiatives.

Responsibilities

• Perform thorough patient assessments to determine eligibility for hospice care and develop individualized care plans.
• Ensure adherence to visit productivity standards and demonstrate understanding of insurance reimbursement processes.
• Complete discharge assessments and maintain accurate documentation within specified timeframes.
• Utilize age-appropriate teaching methods tailored to patients and families, documenting educational tools utilized.
• Manage case responsibilities by developing, implementing, and evaluating care plans based on evidence-based practices.
• Attend interdisciplinary team meetings biweekly to discuss patient progress and adjust care plans as necessary.
• Foster a safe working environment by adhering to infection control protocols and safe medication management practices.
• Engage in performance improvement activities and provide mentorship to new staff as requested.

Required Experience

• Minimum of two (2) years of experience as a Registered Nurse in a home hospice environment.
• Experience using an Electronic Medical Records (EMR) system.
• Excellent critical thinking and communication skills.

Certifications/Licensure

• Current Registered Nurse (RN) License in New Jersey.
• Valid New Jersey driver's license.
• BLS certification.
