Job Description
The Licensed Practical Nurse (LPN) will provide direct and indirect skilled nursing care to residents in a long-term care setting. The LPN will assess and collect data relevant to the residents' physical and psychosocial conditions, while working collaboratively with a Registered Nurse and interdisciplinary team to develop and implement age-appropriate and culturally relevant care plans tailored to adult and geriatric populations.
Responsibilities
- Collect ongoing data relevant to residents' physical and psychosocial conditions and communicate findings to the RN and team.
- Contribute to creating culturally relevant care plans involving residents and their families.
- Recommend revisions to care plans based on resident evaluations and outcomes.
- Maintain knowledge and competency in nursing practice through continuing education and in-service programs.
- Provide leadership under the direction of the RN in the professional practice setting.
- Ensure compliance with safety and cost-effectiveness in care delivery.
Required Experience
Minimum of 1 year of experience as a Licensed Practical Nurse in a long-term care facility.
Certifications/Licensure
- Active New York LPN Licensure
- BLS certification (AHA or RQI)
