Job Description

The Licensed Practical Nurse (LPN) will provide comprehensive care in a Long Term Care facility. Responsibilities include performing assessments, administering treatments and medications, and ensuring proper notifications to the Medical Doctor or Nurse Practitioner regarding pertinent information. The role will also encompass admitting and discharging patients, assisting with activities of daily living (ADLs), as well as specialized care such as gastrostomy tube management, urinary catheter maintenance, nebulizer treatments, wound vac care, and CPAP care.

Responsibilities

• Conduct patient assessments and document findings.
• Administer treatments and medications as prescribed.
• Notify MD/NP of significant patient information.
• Manage patient admissions and discharges seamlessly.
• Assist residents with activities of daily living (ADLs).
• Provide specialized care for gastrostomy tubes and urinary catheters.
• Administer nebulized treatments and wound vac care.
• Ensure compliance with health and safety regulations.

Required Experience

Experience working in a Long Term Care setting is required.

Certifications/Licensure

Current and valid LPN license is required.

Proof of flu vaccination or approved medical/religious exemption must be provided prior to contract start date.
