Full-time evenings, 5x8 per week. Every other weekend and holiday required.

Job Description

The Licensed Practical Nurse (LPN) provides both direct and indirect skilled nursing care to residents in a long-term care setting. The LPN demonstrates the necessary knowledge and skills to care for adult and geriatric patients while collaborating with the Registered Nurse and other healthcare team members to ensure quality care. The position includes the assessment of patient status, involvement in care planning, and participation in maintaining a safe and therapeutic environment.

Responsibilities

• Ongoing collection of data related to residents' physical and psychosocial conditions, communicating findings to the RN and interdisciplinary team.
• Contributing to the creation of age-appropriate and culturally relevant care plans, engaging the resident/family in the process.
• Recommending revisions to care plans based on resident evaluations and collaborating with team members to address care needs.
• Maintaining current knowledge and competency in nursing practice, participating in mentoring and orientation of staff.
• Considering safety, cost, and impact while planning care delivery under RN supervision.

Required Experience

1 year of experience serving as an LPN in a long-term care facility is required.

Certifications/Licensure

• Must possess a valid New York LPN Licensure.
• Basic Life Support (BLS) certification from AHA or RQI is required.
