Schedule/Shift

AM 5 × 8-hour shifts, Monday through Friday. Shifts start at 7:00 AM and end at 3:00 PM.

Job Description

As a Registered Nurse in Home Health, you will provide high-quality nursing care to patients in their homes. You will be responsible for patient assessments, developing care plans, and coordinating care with other healthcare professionals. HomeCare HomeBase experience is required for this position. The coverage area includes Marietta/Kennesaw and Smyrna, with the possibility of assisting in other areas within a 60-mile radius as needed.

Responsibilities

• Conduct Start of Care assessments, Recertifications, and Discharges.
• Perform routine visits and ensure comprehensive patient care.
• Document patient progress and keep accurate records using HomeCare HomeBase.
• Coordinate with physicians and other healthcare providers to develop effective care strategies.
• Ensure compliance with healthcare regulations and best practices.

Required Experience

Recent home health experience is required. RNs must achieve a minimum of 30 points, with specific points assigned for various types of care tasks (Start of Cares 2.5 points, Resumption of Cares 2 points, Recerts 1.75 points, Discharges 1.5 points, and routine visits 0.9 points).

Certifications/Licensure

A valid Registered Nurse (RN) license is required. HomeCare HomeBase experience is mandatory.
